PURPOSE OF THIS FORM

For the authorised signatories of the below named client to request additional sub account/s to be established with the required access as specified in Section 2 of this authority.

INSTRUCTIONS FOR COMPLETION

• All fields are mandatory and must be completed by two account signatories on the account before returning the form to the CDF.
• Digital signatures are not accepted. Form to be authorised with wet ink signatures.

IMPORTANT INFORMATION

• Money is deposited as a loan to The Corporation of the Roman Catholic Archdiocese of Canberra-Goulburn
• The CDF does not offer overdraft facilities. If there are insufficient funds to honour a debit, the CDF will automatically transfer the required amount from the customer’s operating account to cover the transaction.
• Personal contact information will only be used for the express purpose of contacting signatories regarding the accounts they are authorised to operate, any transactions on those accounts or accessing the accounts via CDF Online.

We, the below authorised signatories approve the establishment of account/s as listed in Section 2 of this Sub Account Request.
